The Role of Hormone Testing
Feb 20, 2026In our last blog, we explored some common tests healthcare providers may recommend during perimenopause, including hormone level tests. Today, I want to dive a little deeper into the topic of hormone testing and explore the question: Is it really necessary?
First, let's recap why hormone testing may be recommended during perimenopause. As you navigate this transitional phase, your hormone levels (particularly estrogen and progesterone) can fluctuate significantly before eventually declining. These hormonal shifts are responsible for many of the common symptoms of perimenopause, such as hot flashes, irregular periods, mood swings, and sleep disturbances.
Hormone testing, usually done through a blood test, can provide a snapshot of your hormone levels at a given point in time. This information can be used to:
- Confirm the diagnosis of perimenopause: Consistently high follicle-stimulating hormone (FSH) levels and/or low estradiol levels can be indicative of the transition to menopause.
- Guide treatment decisions: Understanding your hormone profile can help your healthcare provider determine whether hormone therapy or other interventions may be appropriate for managing your symptoms.
- Monitor the effectiveness of treatment: If you start hormone therapy or another treatment, follow-up hormone testing can help assess whether it is having the desired effect on your hormone levels and symptoms.
So, is hormone testing necessary for every woman going through perimenopause? The answer is: that it depends.
When Hormone Testing May Not Be Necessary

- Atypical symptoms: If you're experiencing symptoms that are unusual or not typically associated with perimenopause (such as severe mood changes, significant weight gain, or very heavy bleeding), hormone testing can help rule out other potential causes and guide appropriate treatment.
- Considering hormone therapy: If you're considering starting hormone therapy to manage your perimenopausal symptoms, your provider may recommend baseline hormone testing to assess your individual hormone profile and guide treatment decisions.
- Monitoring hormone therapy: If you do start hormone therapy, follow-up hormone testing can help ensure that your hormone levels are in the desired range and that the treatment is effectively managing your symptoms.
- Assessing fertility: If you're in your late 30s or early 40s and still hoping to conceive, hormone testing, particularly anti-Müllerian hormone (AMH), can provide valuable information about your remaining egg supply and fertility potential.
Making Informed Decisions
Ultimately, the decision to pursue hormone testing during perimenopause should be a collaborative one between you and your healthcare provider. It's important to have an open and honest conversation about your symptoms, concerns, and goals, as well as the potential benefits and limitations of hormone testing.
Remember, hormone levels can fluctuate significantly during perimenopause, so a single test may not always provide a complete or definitive picture. Your provider may recommend repeating tests over time or using hormone testing in conjunction with other diagnostic tools (such as ultrasound or an endometrial biopsy) to gain a more comprehensive understanding of your reproductive health.
